Procurement

The Gulf Coast Center endeavors to contract with providers to serve Galveston and Brazoria County citizens who have mental illness, mental retardation and substance use issues. Contracts for community-based services as well as other goods and services are generally acquired through a Request for Proposal (RFP), Request for Bid (RFB) or the Open Enrollment process.

Open Enrollments

While the Center does publish its Request for Applications at least once every two years as required, Qualified providers are encouraged to submit applications at any time in one or more of the following open enrollment categories for which they are qualified.

Dear Potential Provider,

Please download the following documents and review the required information to be submitted.

GCC FORMS TO BE COMPLETED (signed/dated) AND RETURNED:

Additional Trainings may be required that are specific to the person to whom you are providing services; for example, High Blood Pressure, Diabetes, Tube Feeding, Transferring(Wheel Chair), etc.

REQUIRED DOCUMENTS TO BE SUBMITTED ANNUALLY OR AS THEY ARE RENEWED OR UPDATED :

  • Copy of Valid Auto Insurance (updated regularly)
  • Copy of Valid Driver’s License (to establish residence)
  • Copy of Social Security Card
  • Copy of High School Diploma, GED
  • Three Letters of Reference (only if no High School Diploma or GED)
  • Email/Home/Cell Phone numbers (updated regularly)
  • Copy of Homeowners/Renter’s Insurance Policy (if you provide respite in your home)
  • Home Insurance Waiver (if you provide respite in your home and you do not have insurance)
